On Monday, January 17, 2011 at 9:37 PM, 14 Companies of Los Angeles Firefighters, 4 LAFD Rescue Ambulances, 1 Urban Search and Rescue Unit, 1 Hazardous Materials Team, 2 EMS Battalion Captains, 4 Battalion Chief Officer Command Teams, 1 Division Chief Officer Command Team, Department of Water & Power, and Department of Transportation, under the direction of Battalion Chief Timothy Ernst responded to a Greater Alarm Structure Fire at 12500 West Raymer Street in North Hollywood.

Thanks to Battalion 12 Chief Officer Command Team's keen awareness while driving through the North Hollywood area, they observed smoke poring out of a 135' X 50' commercial building. Quickly notifying Operations Control Division (LAFD's 9-1-1 dispatch center), additional firefighters immediately responded.

Routine firefighting operations such as Forcible Entry and Fire Attack rapidly commenced. The bulk of the flames were toward the front of the 6,600 square-foot building doing business as Star Marble & Tile. As Truck Companies ordered "all ladders off the truck", firefighters were able to climb their way to the conventional, gable truss roof and perform vertical ventilation. A small diameter hose line was also utilized atop the roof to extinguish flames.

Shortly thereafter, firefighters inside made their way through dark, smoky conditions and located the seat of the fire which was an automobile fully involved in flames. Although almost 100 firefighters responded, the first arriving 51 were able to fully extinguish the blaze in just 16 minutes, before anyone was injured. Dollar loss from the fire has been estimated at $65,000 ($40,000 structure & $25,000 contents). The suspected cause of this late evening blaze is categorized as "arcing".

On Monday, November 15, 2010 at 6:09 PM, 13 Companies of Los Angeles Firefighters, 3 LAFD Rescue Ambulances, 1 Arson Unit, 2 EMS Battalion Captains, 2 Battalion Chief Officer Command Teams, 1 Division Chief Officer Command Team, under the direction of Battalion Chief Kevin Pearson responded to a Structure Fire at 6310 Avalon Boulevard in South Los Angeles.
 
Firefighters arrived within four minutes to discover fire showing from the roof of a one story, 100’ x 300’ commercial building. Firefighters used rotary saws to force entry through rolling steel doors, allowing their colleagues to extended hand-lines inside. Vertical ventilation was performed on the conventional construction sawtooth roof to remove smoke and heat. The seat of the fire was difficult to locate due to decreased visibility and an abundance of wood-furniture, paint and additional contents. The bulk of the flames appeared to be in a spray-booth and quickly traveled to the roof. This 71 year-old building was equipped with fire sprinklers which assisted holding the flames in check and aided 80 firefighters to extinguish the fire in just 27 minutes. A division wall prevented the flames from spreading horizontally to adjacent units. Fire loss to the A & S Furniture Manufacturers was limited to $125,000 ($100,000 contents and $25,000 structure). One firefighter sustained a minor injury and was transported to Harbor-UCLA Medical Center in good condition. No civilians were injured. The cause of this fire is undetermined.

On Friday, July 30th, 2010 at 6:51 AM, 16 Companies of Los Angeles Firefighters, 11 LAFD Rescue Ambulances, 3 Arson Units, 1 Urban Search and Rescue Unit, 1 Rehab Unit, 2 Hazardous Materials Teams, 3 EMS Battalion Captains, 3 Battalion Chief Officer Command Teams, 1 Division Chief Officer Command Team, 2 bull dozers, LAPD, So Cal gas company, dwp, DOT, building & safety, LA County Health Services, Cal Osha, along with other agencies, all under the direction of Assistant Chief Daniel McCarthy responded to a MAJOR EMERGENCY at 936 East 59th Street in South Los Angeles.

Firefighters arrived to find a 100’ X 100’ one-story commercial building that sustained a massive explosion, which shook windows of nearby businesses. The front one-third of the building and roof collapsed in addition to being well involved in flames. One victim was thrown into the street from the explosion and pinned under a vehicle. Another victim was found in Cardiac Arrest after being electrocuted from power lines that were down. Emergency Traffic was declared over the radio by the Incident Commander to alert all on scene of the electrical hazard.

Firefighters jumped into action, and made a quick aggressive entry into the collapsing building. They endangered their lives to perform a rapid search of this large welding business, in hopes to rescue anyone possibly trapped. No one was immediately found, and due to the structure tumbling down around them, the Firefighters had to exit and battle the blaze defensively. Over 100 Firefighters were able to successfully knockdown the flames in just 25 minutes.

The Urban Search and Rescue Team, was then able to eliminate hazards and shore up beams, allowing a safer environment to perform a detailed and thorough search. K9 units were utilized to locate anyone possibly trapped under the rubble, however there were no hits and no additional victims were found.

Sadly one adult male deceased from electrocution. A second adult male suffered critical burns and was in the ICU at a California hospital awaiting a transfer to LA County USC hospital.

The cause of the explosion was due to illegal tampering with the gas meter. This enclosed structure was likely filling with natural gas, and when an ignition source sparked it, the powerful and deadly explosion occurred. Monetary loss from the explosion is estimated at $250,000 ($175,000 structure & $75,000 contents).

County Health and a Building and Safety Inspector, under the direction of Assistant Chief Jeffery S. Mottram, responded to a Greater Alarm Structure Fire at 9044 N. Woodman Av. in the Panorama City area.

First arriving Firefighters on scene found heavy smoke coming from the rear of a 1 story, 100 foot by 100 foot commercial building. Further investigation indicated the fire was in the occupancy located in the center of the small, strip mall type complex. As Firefighters vigorously deployed handlines to gain control of the fire, the multiple heat holes cut by the ventilation team spewed fire, indicating the fire had truly progressed into the attic. The IC was prompted to request additional companies to gain control of the fire. A consideration was made to go defensive with master streams, but the deployment of additional 2-1/2 inch hoselines by Firefighters from the added incoming companies, thwarted the progress of the fire by blasting out the drop ceiling and reaching the seat of the attic fire. Members were alerted that a large portion of the roof was unstable and at 2302 hours the majority of the roof collapsed.

It took 68 Firefighters 37 minutes to achieve a knockdown on the fire. There was one minor injury to a Firefighter who was transported to an area hospital. The cause is currently still under investigation. The estimated dollar loss is $600,000 [$200,000 structure, $400,000 contents]. The occupancies effected were: the AlRight Pawn Shop (point of origin), Pupusas Restaurant and the Iglesia Misionera.

On Friday, April 10, 2009 at 2:35 PM, 10 Companies of Los Angeles Firefighters, 2 LAFD Rescue Ambulances, 1 Arson Unit, 1 Hazardous Materials Team, 1 EMS Battalion Captain, 1 Battalion Chief Officer Command Team, 1 Division Chief Officer Command Team, under the direction of Battalion Chief Dan Mc Carty, responded to a reported Structure Fire at 7245 N. Laurel Canyon Bl. in the North Hollywood area.

Firefighters responded to reports of a structure fire at a known address approximately a block from the Fire Station. As Firefighters pulled out of the station, they saw a huge plume of thick, black smoke. On arrival, Firefighters were directed to a 200 gallon tank, with approximately 40 gallons of fuel, which had ignited. The intense fire was impinging on two, 8,000 gallon containers of asphalt contents, prompting Firefighters to expeditiously deploy handlines as cooling measures for the larger containers, preventing their potential ignition. An immediate request for additional companies, a Foam Tender and a Hazardous Material Squad was made by the IC.

With copious amounts of water, Firefighters were able to gain control of the blaze. The intense heat from the blaze did partially compromise one of the 8,000 gallon containers. Because of the abundance of water needed for the fire, hydrocarbon tainted runoff was diked to prevent flow into the storm drain system.

64 Firefighters were needed to knock down the fire in 17 minutes. No injuries were reported to civilians or Firefighters. The cause is undetermined and the dollar loss is estimated at $500,000 to machinery and wiring. The asphalt plant is one of two plants in the City of Los Angeles, a part of the Department of Public Works.

County Health Haz Mat and LAPD Haz Mat, under the direction of Battalion Chief Peter Benesch, responded to a Hazardous Materials Incident at 2210 S. Barry Av. in the Sawtelle area.

Firefighters responded to reports of a hazardous material spill at a commercial business. When Firefighters arrived, they found 25 to 30 employees of the business self evacuating and were promptly taken to a safe location. Further investigation revealed the company, Barry Avenue Plating Company, had contaminated soil in the process of being removed from the property. Approximately 200 gallons of Calcium Polysulfide, used in preparing the soil for removal, was accidentally spilled. The chemical is not used in plating operations.

A Haz Mat entry team deemed the material to be static and contained in a 20 foot by 10 foot containment pool. Readings were taken and no atmospheric hazard was found. The incident was downgraded and turned over to L.A. County Health Haz Mat to oversee the proper clean-up and removal of the material.

52 Firefighters were utilized during the incident. No residential evacuations were necessary, but Olympic Boulevard was shut down between Barrington Avenue and Federal Avenue for the duration of the incident.
